Print and fill out the fax-in membership form, check the box indicating you agree to the terms of use and print clearly:


• what card you are using:

• the first and last names on the credit card, the credit or debit card number and expiry date, and the billing address for your payment card if different from your mailing address
• what username and password you wish to set

Your membership will be active once your payment is processed. You will receive a welcome e-mail instantly. US members allow 4-6 weeks to receive your free alternative health care benefit in the mail.

Please allow 2 weeks for processing. Your membership will be active once your payment is processed. You will receive a welcome e-mail and if a US member your free alternative health care benefit in the mail. Professional Members will also receive the Welcome Package in the mail and their website seal.
